A major study published in The Lancet Public Health has found that physical activity performed as part of a job—such as heavy lifting or strenuous labor—may increase dementia risk by 20%, while leisure-time exercise like running, swimming, or cycling is associated with a 24% lower risk of the condition.
Researchers from the University of Southern California analyzed data from 74 studies involving over 4.2 million participants aged 45 to 93, tracked over approximately a decade. The study is the first to systematically examine whether the so-called ‘physical activity paradox’ applies to dementia, a phenomenon previously observed in cardiovascular research.
Key Findings
The research distinguished between occupational physical activity and leisure-time physical activity, revealing starkly different associations with dementia risk. Those who engaged in higher levels of leisure-time exercise had a significantly reduced risk of dementia, including Alzheimer’s disease and vascular dementia. Conversely, jobs requiring prolonged standing, heavy lifting, or strenuous movement were linked to an increased dementia risk.
The study also noted a modest increase in dementia risk among people who actively commute, though researchers cautioned that this finding was based on limited data and remains uncertain.
Researchers’ Interpretation
The study’s lead author, Professor Natan Feter, suggested that the observed associations may reflect cumulative effects of social, economic, and environmental factors rather than the physical activity itself. He noted that occupational physical activity often occurs in environments with greater exposure to air pollution and psychosocial stressors, which could contribute to higher dementia risk.

Dementia remains a leading cause of disability globally, with 14 modifiable risk factors—including physical inactivity—accounting for 45% of cases.
